Job Summary:   The Med-Surg/Tele/ICU Educator is accountable for providing educational expertise in medical-surgical/telemetry and intensive care nursing as well as assisting with education throughout the organization. As a recognized expert, the Med-Surg/Tele/ICU Educator acts as a mentor to the nursing staff, providing consultation and education. The Med-Surg/Tele/ICU Educator is responsible for the ongoing review, revision, and management of staff clinical competencies. Further, the Med-Surg/Tele/ICU Educator is responsible for conducting competency evaluations and for coordinating opportunities to match staff learning needs with patient care needs. The Med-Surg/Tele/ICU Educator does not assume responsibility for an assigned case load of patients, but is required to maintain clinical competence by working directly with nursing staff in the assigned patient care areas, on a routine basis. Position Summary: The Coder III is a member of the Health Information Management Team responsible for ensuring the accuracy and completeness of clinical coding, validating the information in the databases for outcome management and specialty registries, across the entire integrated healthcare system. Job Summary:   The priority goal for the FOCUS (Frontlines of Communities in the United States) Patient Navigator is to provide linkage to care for patients diagnosed with Human Immunodeficiency Virus (HIV), Syphilis, Hepatitis B (HBV) and/or Hepatitis C (HCV), where they can receive appropriate levels of care with the desired outcome being improved health and quality of life for designated patient populations. The navigator will promote patient informed decision making and identify/minimize socioeconomic, environmental, and circumstantial issues that bar access to care and hinder compliance with treatment. He/she will coordinate care plan adherence activities such as appointment reminders, arranging transportation and coordinating the referral process between St. Joseph's Medical Center (SJMC), community health centers/primary care clinics and social supportive resources. The process involves a high level of interaction with SJMC staff, patients and community health providers to determine optimal care needs and settings and transfer relevant patient health data via a secure web-based system and maintain patient documentation on every patient contact, as well as necessary follow-up to ensure continuity of care. This individual must have excellent communication skills including the ability to communicate sensitive issues, including sexuality, drug use, and physical and mental health. This position requires the full understanding and active participation in fulfilling the Mission of the Organization.
