Position Summary:
Upon referral provides professional Physical Therapy services including evaluations, planning individual patient care programs, and treatments. Demonstrates/uses current knowledge of principles and practices of Physical Therapy. Participates in job related departmental functions as an integral member of the patient care team.
Minimum:
- Minimum of one year experienceas a physical therapist in a physical disabilities setting, preferably in acute care or acute rehab, OR completion of six (6) to (9) months internship in an acute care or acute rehab.
- Knowledge of physical therapy theory, treatment, and evaluation in areas of neurology is recommended.
- Doctorate, Masters or Baccalaureate degree from a school of Physical Therapy accredited by the Council on Medical Education and Hospitals of the American Physical Therapy Association or its equivalent.
- Current AHA BLS certification required
- Current and valid physical therapy license, according to the laws of California.
- CCS paneling for pediatric care (only if they are treating pediatric patients).

About the Position

performs all phases of physical therapy treatment and documentation; performs other tasks as assigned related to quality of care and effective department function. Under the guidance and direction of a Physical Therapist: participates in the treatment program planning; administering therapy modalities, procedures and tests; documentes accurate patient records and progress notes; maintaines appropriate oral or written contact with the supervising PT, co workers, and the public. Ensures a safe work environment by being alert for unsafe conditions in the home and gives appropriate instructions for their correction; Assists Physical Therapy Supervisor with Q.I. as required. Performs other related duties such as participating in departmental functions and activities as required, including in-service education and clerical and other tasks relevant to effective department function as necessary.

Gilbert Urgent Care (GUC) is a member of the Dignity Health organization which includes St. Joseph hospital, Chandler Regional Medical Center and Mercy Gilbert Medical Center. Gilbert Urgent Care is located at 1501 N Gilbert Rd, Gilbert, AZ 85234.
GUC is an urgent care that offers many services, including non-emergency illnesses, non-emergency injuries, and allergic reactions. This GUC is one of the best in Arizona. As part of the Dignity Health network, Dignity Health Urgent Care in Gilbert is dedicated to delivering high quality, compassionate care and access to Gilbert and nearby communities. GUC is open 365 days a year from 7am – 10pm.
Provides excellent care, in the role of Staff Physician, for the patients of Dignity Health Urgent Care Clinics. Privileged or eligible to obtain privileges at CRMC/MGMC. Experience in the evaluation and treatment of acute medical conditions and injuries common to patients of all ages (e.g. chest pain, abdominal pain, infections, laceration repair, orthopedic injuries). Experience in utilizing and interpreting common lab tests, EKGs and plain X-ray studies.
Located in the Town of Gilbert, AZ, a suburb of Phoenix, this position is critical to the success of Chandler Regional Medical Center and requires the full understanding and active participation in fulfilling the Mission of Dignity Health.
