Position Summary:
Provides physical therapy techniques to relieve pain, develop or restore functions using physical means according to the plan of care.
The incumbent demonstrates competency in patient care for neonate, pediatric, adult and geriatric patients. May be required to direct the work of a Physical Therapy Assistant.
The Therapist provides patient care, including evaluating the patient for treatment, providing and directing administration of the care plan upon the referral from the physician. Consults with other health care disciplines to coordinate therapeutic programs and discharge planning for individual patients. Instructs patients and caregivers in home program exercises and functional activities. Assists with patient program management, while delivering quality patient services.
Chandler Regional Medical Center Outpatient Rehabilitation Services is seeking an experienced women’s health pelvic floor Physical Therapist with excellent patient experience behaviors.
The ideal candidate should be well-rounded in the evaluation and treatment interventions for control of pelvic pain, reduction of muscle spasms, postural education, increasing strength, neuromuscular endurance improvement, dynamic stabilization, general relaxation, coping strategies, and wellness promotion for gynecologic, urologic, musculoskeletal, and gastrointestinal disorders.
Completes the duties of a California licensed Physical Therapist in compliance to California licensing standards and within the scope of clinical skills required for the patient population being treated and as determined by patient age and diagnosis. Performs the clinical duties of patient evaluation, care planning, treatment intervention, patient/family education, and discharge planning within physical therapist clinical practice standards. Reports directly to the department Coordinator. Assists in process improvement and other department operations as requested by the department coordinator. Ability to write reports and procedures and document in the medical record. Ability to effectively present information and respond to questions from co-workers, managers, patients, physicians, family members and the general public. Ability to calculate figures and amounts such as weights, dosages, flow rates, proportions, percentages and volume. Ability to apply concepts such as fractions, percentages, ratios, and proportions to practical situations. Ability to define problems, collect data, establish facts, and draw valid conclusions. Ability to interpret an extensive variety of technical instructions in mathematical or diagram form and deal with several abstract and concrete variables. Must have ability to critically think and problem solve. Must have the ability to read, write and speak English.. The Home Health Physical Therapist will work together with a multi-disciplinary team to provide care, identify functional goals and outcomes, and increase independence for our patients and their families. Home physical therapists specialize in aiding patients who have limitations in function by examining and evaluating the patient to develop and execute a plan of care in collaboration with the physician.
Home health physical therapists will have the ability to work autonomously, and participate in care coordination with the treatment team, physicians, and referral sources. The physical therapist will work with patients, their families and the care team to achieve recovery and to re-engage the patient with their community.

Position Summary:
Upon referral provides professional Physical Therapy services including evaluations, planning individual patient care programs, and treatments. Demonstrates/uses current knowledge of principles and practices of Physical Therapy. Participates in job related departmental functions as an integral member of the patient care team.
Minimum:
- Minimum of one year experienceas a physical therapist in a physical disabilities setting, preferably in acute care or acute rehab, OR completion of six (6) to (9) months internship in an acute care or acute rehab.
- Knowledge of physical therapy theory, treatment, and evaluation in areas of neurology is recommended.
- Doctorate, Masters or Baccalaureate degree from a school of Physical Therapy accredited by the Council on Medical Education and Hospitals of the American Physical Therapy Association or its equivalent.
- Current AHA BLS certification required
- Current and valid physical therapy license, according to the laws of California.
- CCS paneling for pediatric care (only if they are treating pediatric patients).

The Physical Therapist evaluates and conducts medically prescribed physical therapy treatment programs in an inpatient rehabilitation unit setting. Supervises physical therapy assistants and aides in performing treatment. Supervises students.
Responsibilities:
- Provides initial evaluation of patient's abilities and designs appropriate intervention plans.
- Treats patients with therapeutic procedures, modalities and or equipment to improve function.
- Assures efficient management of work processes, by completing tasks and assignments by time standards.
- Completes post-intervention activities: session closure, documentation, communication, charge capture.
