Position Summary: This is a full-time position working for 2 important programs.   The first is for the Outpatient Diabetes program for 20 hours per week. The job duties include: Answering and greeting patients and the public by phone and in-person, scheduling patient appointments, coordinating and accepting referrals from the Hospital and MD offices, insurance verification, billing, and coordination with the licensed staff. The position is customer service oriented and requires compassion, confidentiality, and strong organizational skills. Must have excellent computer skills and will work independently. Experience working in multiple software systems is a must since Cerner, MS4 and Octavia are regularly utilized. The position is supervised by the Manager of the Transitional Care Center.   The other 20 hours per week are dedicated to the Outpatient Palliative Care Program, based in the same office on Palisades street in Santa Maria.
