SUMMARY - In collaboration with the physician/medical director, assumes clinical responsibility for tasks performed within Nurse Practitioner's scope of practice. Including: taking patient's history, performing physical examinations, ordering appropriate diagnostic tests, establishing diagnosis and provide care and management for common illnesses and disease prevention in the pediatric and adult patient. The NP may assist in the admission, transfer or discharge from Health Care System. Makes appropriate referrals to specialists and community agencies. Participates in quality assurance and quality improvement program. Works within multidisciplinary team to develop plan for the patient and their family. In addition to the routine duties of detailed in NP scope of practice the in-patient, specialty NP will provide care and management for acutely ill patients, respond to emergency situations, and may also see patients in the intensive care units. Also responsible for the management of the standards of care, quality, national certifications, and representation to key stakeholders for the specialty program.
Job Summary:
The Nurse Practitioner is a registered professional nurse with advanced skills, employed by the Medical Center, for the purpose of enhancing the quality of care for patients. The Nurse Practitioner is accountable for ensuring high quality cost-effective care for the departments patients. This responsibility includes the utilization of specialized advanced clinical nursing skills and knowledge in the areas of direct and indirect patient care, clinical nursing research, consultation, and the education of patients, families, and personnel. The Acute Care Nurse Practitioner (ACNP) for in collaboration with physicians, is responsible for performing medical examination, diagnosis of medical conditions, and managing phases of cardiovascular care. The ACNP for SJMC is a nurse practitioner who is Master's prepared and focuses on the care of acutely ill, patients with multiple complex problems. The ACNP for SJMC has extensive experience and expertise in assessing and managing acutely ill patients from specialty populations, including critical care and cardiovascular patients. The attending physician supervises the nurse practitioner's clinical practice. Responsibilities include the coordination of care with other members of the health care team to provide quality care and patient education and to minimize system variances. The Nurse Practitioner participates in collecting, collating, and analyzing data to identify trends and opportunities to improve care, patient information, and/or programming enhancements. The Nurse Practitioner is a mentor who demonstrates ability in providing educational experiences and communicating knowledge of patient care to members of the interdisciplinary team, patient, and families. This position requires providing care/service to an acutely ill through rehabilitating patient population in a manner that demonstrates an understanding of the functional/developmental age of the individual served. This position requires the full understanding and active participation in fulfilling the Mission of the Organization. In keeping with the Pacific Central Coast Health Center's philosophy, the Walk-In Care Nurse Practitioner is responsible for providing direct patient clinical care in a walk-in setting and provides health maintenance exams with appropriate patient education for all age groups, including well child exams, adolescent exams and sports physicals, and adult physicals including pap and pelvic exam. Nurse Practitioners will work with direction and supervision from Physicians.Knowledge of nurse practitioner principles and techniques.
