Position Title:Supervisor of Rehabilitation Services
Position Summary:
Plans, develops, implements and integrates the delivery of Physical, Occupational and/or Speech Therapy in the outpatient and/or acute care settings for patients requiring either single discipline or multidisciplinary care from the Rehab team. Oversees quality, patient experience, regulatory and billing compliance, business development and all aspects of staff development and employee engagement..
- Develops and maintains core competencies and behavioral skill sets that support exceptional patient and co-worker experiences, consistent with Dignity Health core values; provides timely and effective direct patient care in a professional, compassionate, and timely manner; identifies and responds to the needs and expectations of all customers, both internal and external.
- Develops and implements policies and procedures to maintain Program compliance with accreditation, certification, licensure, government, and other regulatory requirements and ensures understanding across staff.
- Identifies and projects present and future Program needs as well as develops new programs or makes program changes as appropriate to ensure quality and competitiveness in community; implements systems to successfully meet reporting requirements/
- Facilitates the development of cross-functional, interdisciplinary, and patient-focused teams; provides effective orientation, training, education, and development of staff so that services are delivered to the satisfaction of patients, physicians, and referrers; directs, evaluates, and supervises Program staff; maintains open communication and collaboration with other departments and individuals in resolving issues affecting the delivery of care.

The incumbent demonstrates competency in physical therapy for pediatric, adult, and/or geriatric patients. The incumbent provides patient assessments, develops and implements individual patient treatment programs, provides therapeutic interventions utilizing current and effective methodologies, and maintains effective documentation.

Position Summary: Reporting to the Administration Supervisor, the Medical Group Coordinator supports the activities of the Professional Corporation (PC) to meet the requirements of the Woodland Clinic Medical Group (WCMG) and outside regulatory agencies. This position supports the WCMG's Medical Director with all aspects of Physician and Allied Health Professional Support.

Coordinates plan of care with the health care team. Assumes leadership role for unit personnel to prepare and assist them with their responsibilities. Employees are responsible for completion of unit competency assessments, annual safety review, CPR, and renewal of license by maintaining continuing education units as required by hospital policy. In addition to these major responsibilities, the Registered Nurse may be assigned other duties.
RN Selection Criteria
- Minimum two years Acute Care Experience (new grads considered on a case by case basis.)
- One to two years ICCU experience preferred.
- Graduate of an accredited RN program.
- Currently licensed by the State of California as a Registered Nurse.
- Current BLS, ACLS certification.

Position Summary:
Working under the Director of Quality Management, the Quality and Safety Nurse has the responsibility to review triggers in the Risk Trigger Monitoring platform and report results to various committees, work groups, physicians and leadership. This position will work extensively with hospital leadership to share findings, confirm solutions and measure results of safety initiatives. This position may be involved in the work groups; however, their primary focus is on the review of triggers and identification of harm and the presenting of information to improve patient outcomes. This position will evaluate care according to evidence based practices.
Core Duties
- Ability to utilize clinical knowledge, skills and expertise to review outliers identified through technology for opportunities to improve patient care, including summarizing opportunities for improvement and intercepting care to improve patient outcomes
- Assists in retrieval of analytics from technology to share results and findings to individuals and groups of leaders who can influence and improve the level of care to patients
- Participate in system level development of technology to reduce harm to patients
- Assume responsibility for other related assignments/duties resulting in improved patient care
- Keep the organization’s mission, vision and values at the forefront of associate decision making and action.
- Ensure that the patient perspective is a driving force behind our actions and business decisions; crafting and implementing service practices that meet patient’s and own organization’s needs. (Focus also includes internal and external customers
- Strong interpersonal and communication skills with the ability to deal effectively with customers, peers and senior executive management and the ability to present reports orally or in written form in a clear and effective manner. A key role of this position is the ability to communicate, engage and partner at all levels of the organization
- Provides leadership and oversight in direct support of the Patient Safety and Quality initiatives under the Risk Trigger Monitoring® program
- Has knowledge of processes, tools, techniques, healthcare environment/delivery for assessing and improving the delivery of safe care to patients
- Has knowledge of and the ability to identify and evaluate actual and potential hazards, and identify potential solutions in partnership with other key stakeholders

Position Summary:
Manages the resources and daily activities of a nursing department while ensuring environmental factors support high standards of patient care. Assists in the training and development of new and existing staff members. Creates and maintains an environment that is conducive to learning transfer. Coordinates the activities of the unit for each shift and directs, organizes, and assigns work to the nursing staff. Provides patient care as needed..
- Schedules nursing staff to specific shifts, taking into account past utilization trends. Measures employee performance. Assists in the training of new staff members and the development of existing staff members.
- Coordinates the activities of the unit during a particular shift and directs, organizes, and assigns work to the nursing staff.
- Assesses, monitors, and educates the nursing staff on patient care. Institutes emergency procedures as necessary.
- Provides nursing care to patients on an as needed basis.

The Manager of Care Coordination is responsible for the oversight of Care Coordination including, but not limited to: clinical resource management, discharge planning activities, patient advocacy, and best practice in medical necessity determination and concurrent review. This position will oversee the department in an efficient manner to ensure timely and compliant care coordination, discharge planning, utilization review resulting in quality patient care. The Manager consults and collaborates with other managers, departments, physicians, administration and healthcare workers regarding departmental activities or initiatives. This position leads or participates in quality and process improvement projects related to care coordination functions. The Manager is responsible for all performance management activities, including hiring, training or disciplinary issues. This position will conduct, lead, facilitate or participate in staff meetings, interdepartmental meetings, post-acute care provider meetings or patient/family meetings as appropriate. The manager will develop or participate in the development of the departmental operating budget, and monitor scheduling and expenditures to verfiy that the department is operating within the established business plan. This position reviews documentation or utilization data for the purposes of reporting, trending, quality improvement or validation of compliance as indicated. The Manager guides department operations according to the organizational objectives, hospital policies, standards of practice and Federal and State Regulations.
