The Phlebotomy Technician will perform venipuncture, heelstick and/or fingerstick for the purpose of collecting blood specimens for laboratory test procedures on neonate, infant, adolescent, adult and geriatric patients. The Home Care Physical Therapist is responsible for providing physical therapy services to Home Care patients.
The Physical Therapist participates as a member of the health care team, working with professional and non-professional staff and associated professionals in providing care which provides optimum home care and continuity of care to referred patients, addressing the age of the patients served.

Ability to write reports and procedures and document in the medical record. Ability to effectively present information and respond to questions from co-workers, managers, patients, physicians, family members and the general public. Ability to calculate figures and amounts such as weights, dosages, flow rates, proportions, percentages and volume. Ability to apply concepts such as fractions, percentages, ratios, and proportions to practical situations. Ability to define problems, collect data, establish facts, and draw valid conclusions. Ability to interpret an extensive variety of technical instructions in mathematical or diagram form and deal with several abstract and concrete variables. Must have ability to critically think and problem solve. Must have the ability to read, write and speak English.. Advanced experience with computer software programs and clinical documentation in Hospital based practice. Multi-lingual, Spanish, Tagalog. LIC PT = Licensed Physical Therapist in State of CA. Under the general guidance of the Director of Rehabilitation Services and Therapy Manager; the Physical Therapist administers patient evaluations, plans and implements patient treatment proficiently for all assigned patient diagnoses. This position supervises Physical Therapist Assistants in performance of clinical duties. The Physical Therapist demonstrates exceptional time management skills in performing all assigned tasks. In addition, the Therapist also demonstrates sound judgment in clinical decision making.
This position assists in supervision, training and orienting new staff (Level II and III only). CCS-paneled Physical Therapists will be responsible for participation in case conferences when appropriate, discharge planning activities, and coordination with the multidisciplinary team/Utilization Review Case Manager for referral to a CCS-paneled provider or CCS Medical Therapy Unit for patients who may continue to require Physical Therapy services after hospital discharge.
