Duties: - Educate ED staff on the role of the ED Specialty SW in order to identify and refer homeless patients to the SW in addition to electronic EHR alerts. - Perform prompt, comprehensive assessments on homeless patients presenting in the ED. - Develop plan for safe return to the community linking the patient to any available resources that support them in overcoming barriers to food, shelter and medical care. - Implement any necessary interventions including but not limited to crisis intervention, abuse screening, mental health assessment, and risk assessment. - Develop relationships with community agencies and organizations that serve the homeless to enable warm handoff whenever possible. - Engage ED providers in providing any supporting care that will enhance the health and housing security of homeless patients. - Maintain data as requested - Participate with SW Manager/Supervisor in measuring and reporting on metrics (see below) *** THIS POSITION IS GRANT FUNDED THROUGH DECEMBER 31, 2021.
