We’re seeking candidates with the following skills and strengths:   - Desire to work collaboratively with interdisciplinary teams to ensure patient safety, positive patient experience, and delivery of quality care - Commitment to extend dignity, caring, and respect to all patients, physicians and family members - Experience communicating effectively both verbally and in writing - Proficiency, efficiency, and thoroughness using electronic medical records - Commitment to patient confidentiality - Maintain knowledge of changes governing Home Health Agencies in accreditation, State and Federal Regulation, third party payors, and patient care needs - Enjoy working a flexible schedule and be willing to work overtime, if needed, for the provision of quality patient care - Enthusiasm about field nursing care, as our patients will span throughout Yolo, Sutter, Yuba, and Solano Counties.  - Knowledge of current nursing care techniques, principles and procedures, The Joint Commission, Title 22, Medicare regulations pertaining to Home Health.   The Registered Nurse coordinates, supervises and implements patient care. Observes and maintains records on patient care, condition, reaction and progress. Conducts initial and ongoing assessment, develops patient care plan, manages patient treatment, teaches patient and family, identifies and corrects variances from care plan and completes discharge planning. Administers prescribed medications, changes dressings, cleans wounds, monitors patient vital signs and provides patient and family education on treatment plan and ongoing care needs. Obtains consults and referrals as necessary. Knowledge of current nursing care techniques, principles and procedures. JCAHO, Title 22, Medicare regulations pertaining to Home Health. Must have ability to: Work as a team member to meet the department goals and objectives. Treat co-workers, patients, physicians and family members with dignity and respect. Maintain patient confidentiality. Communicate effectively with team members in verbal and written form, using electronic clinical information system. Establish and maintains a cooperative working relationship with physicians, nursing staff, therapist, other hospital departments, and outside agencies. Position Summary:   The Home Health Skilled Nurse will work together with a multi-disciplinary team to provide care, identify care needs, goals and outcomes, and increase independence for our patients and their families. Home Skilled Nurses specialize in providing care in a home setting for patients with a variety of care needs and acuities by examining and evaluating the patient to develop and execute a plan of care in collaboration with the physician.   Home health Skilled Nurses will have the ability to work autonomously and participate in care coordination with the treatment team, physicians, and referral sources.
