 The Arizona Care Network (ACN) is a clinically integrated physician network (CIN) established as a result of a Dignity Health and Tenet Health physician collaboration.  The Embedded Care Specialist is an employee of  Dignity Health Management Services Organization.   The Embedded Care Registered Nurse (ECRN) is an individual with significant experience and demonstrated ability to engage providers and care teams in collaborative care, generation of better clinical outcomes, driving higher patient satisfaction with service delivery, and efforts to improve the cost-efficiency and value of service delivery.   In this role the ECRN supports the community clinical care teams in such a manner that the ECRN becomes an integral part of the local care team (either in-person or virtually) providing care coordination under the supervision of the patient’s personal clinician.   The ECRN also represents to the community clinical care teams ACN’s significant Population Health Management (PHM) capabilities in order to allow for the expansion and intensification of PHM activities within all care locations across the network, to include ambulatory and inpatient facilities. In this role the ECRN will provide actionable information that will allow each care team to take advantage of the risk-segmentation, risk-stratification, care gap analysis, clinical and financial benchmarking, and specialized Care programs and campaigns made available by ACN to all its collaborating clinical sites.    The ECRN works under the direct supervision of the ACN  Manager Embedded Care with the ACN Chief Medical Officer having direct oversight of the clinical integrity. The RN will be working in the community, hospitals, provider offices, skilled nursing facilities, the ACN Command Center and other embedment opportunities to support patient care as available.   Plans and coordinates all phases of skilled nursing care coordination using a problem solving process that includes assessment, problem identification, goal definition, plan development, evaluation to achieve optimum patient outcomes.  This position requires a successful track record of interaction with physicians, facility nursing staff, patients and their families, as well as all aspects of patient care management including referral development, assessment of patient clinical needs, and coordination of the interdisciplinary plan of care.     PRINCIPLES DUTIES AND RESPONSIBILITIES - Concurrently reviews patient’s records to collect data to carefully understand the needs of the patient by scrutinizing their background history, understanding their current needs, and arranging for their wellbeing. - Using industry guidelines, assesses appropriateness of admission, level of care, and length of stay. - Coordinates with other disciplines to facilitate the patient’s individual needs. Makes plans to resolve unexpected care requirements.   Anticipates and identifies variances in the care process related to those identified needs. - Assists in development, implementation and revision of individual treatment plans; assures that services provided are specified in the Treatment Plan and monitors progress toward treatment goals, including documentation of daily improvement in patient’s condition or otherwise notes lack of improvement for reassessment of appropriateness of treatment plan. - Communicates with the nursing home physicians, regularly, to evaluate the status of each patient. Collaborates with other team members to ensure appropriate interventions are implemented.  These communications will be needed as frequently as is need to ensure care is appropriate according to patient status. - Coordinates with other disciplines to facilitate the patient receiving the required care at the expected time including plan of care to reduce incidence or re-admission to acute care setting, including physical, occupational and rehabilitative therapy. - Coordinates transfers to a lower level of care, home health referrals, and durable medical equipment delivery to facilitate discharge from skilled nursing facility. - Measures effectiveness and outcomes of the care plan and collaborates with the health care team for quality improvement. - Interacts with patient and family providing transition plan for treatment goals and post-discharge needs. - Assesses and makes referrals to appropriate community resources to facilitate patient progression toward expected goals/outcomes. - Has a working knowledge of the financial aspects related to a variety of payer sources. - Reports weekly to the Director of Embedded Care or Medical Director for Quality and Utilization regarding patient status and identifies any potential risk management. - Maintains case files and reports.

The Social Worker is a key liaison between the primary care team and behavioral health team, and will work very closely with the medical providers, behavioral health counselors, care coordinators, and patients to meet the patient’s multiple clinical and non-clinical health needs through a strength-based model.  The Social Worker will be working in the community, medical offices, skilled nursing facilities, hospitals and the ACN Command Center.   PRINCIPLES DUTIES AND RESPONSIBILITIES - Maintains a thorough knowledge of social work principles and practices, including case management, individual and group counseling techniques, diagnosis and treatment of mental illness. - Performs intake, mental health screening, assessments and diagnoses for clients and creates strength-based patient care coordination plans with multi-disciplinary team members. - Documents in the clinical record, comprehensive assessment of patient needs including barriers to timely treatment, clinical interventions, support and appropriate community resources. - Provides clinical oversight to ensure provision of all covered services identified on the service plan; referrals to community resources as appropriate; and continuity of care between inpatient and outpatient settings; services and supports, as applicable - Develops and implements individual plans of treatment which assist patients and families to cope and/or restore social, emotional, financial and environmental factors which affect and/or are affected by illness. - Assists patients to receive mental health services, health education and direct counseling services. - Refers participants and their families to appropriate community agencies or facilities; acts as liaison with such organizations and advocates for patients and/or their families. - Performs ongoing case management, which may include interacting with police, psychiatric services, and other community agencies and staff. - Identifies medical aspects of anticipated care needs, determining when and if a Registered Nurse, Pharmacist, Diabetic Educator or Primary Care Physician is needed. - Ensures the development and implementation of transition, discharge and aftercare plans prior to discontinuation of behavioral health services. - Prepares complete and concise written materials, reports, and documentation. - Assists patients in achieving compliance and improving adherence to plan of care, notifying primary care physician of issues. - Actively participates in internal quality improvement teams and work with members proactively to drive quality improvement initiatives in accordance with the mission and strategic goals of the organization, federal and state laws and regulations, and accreditation standards, when assigned. - Notifies supervisor of crisis situations, unusual incidents, and/or on-going problems within appropriate timelines, including reports to outside agencies, as appropriate. - At all times demonstrates cooperative behavior with supervisors, subordinates, colleagues, clients and the community. - Other duties as assigned by Supervisor.

The position is also responsible for assisting members with choosing a PCP and or Specialist when requested, help manage the referral process including the transfer of medical records and when requested, assist in scheduling appointments to appropriate Care Coordinators based on patient need. The position requires excellent listening skills, the ability to research member and provider questions, and to provide timely follow-up ensuring that all questions or concerns have been fully addressed. The Connect Services Navigator must at all times demonstrate a supportive demeanor while maintaining a positive, caring and professional attitude.     PRINCIPLES DUTIES AND RESPONSIBILITIES   - Connect Services Navigator is the customer service provider for payor-employer contracts; the liaison for plan members who connects them with other plan resources to ensure their needs and expectations are met. - Provides first line response for member and provider inquiries and requests including welcome calls to new product members, requests for services and referrals. - Completes data entry and information updates in ACN member database. - Verifies benefit eligibility and prior authorization, assists member in reviewing their benefit choice as necessary. - Assists members in finding a PCP or specialists including transition of medical records and schedules appointments as requested - Assists members in obtaining same day access to care when appropriate. - Makes post discharge Emergency Department outreach follow up calls as necessary. - Acts as a first line response for phone or electronic referrals for the Care Coordination team. - Receives incoming referral requests information, processes the referral and assigns patient to appropriate care coordination team or disease management program. - Provides disease management or other educational materials as requested. - Conducts necessary research for requests and identifies appropriate contact for transfer of calls. - Provides follow up with members to assure satisfaction with resolution to queries. - Sends outbound mailings including Welcome Packets, care coordination related letters and community resource information to members. - Completes all care coordination tasks assigned by ambulatory or transitional team members. - Customer Advocates for commercial insurance and employer self- insured programs. - Processes all transitions of care, out of network referral and complex issue resolution for plan programs to appropriate care coordination team member. - Run daily and weekly reports for clinical integration contract patients including error messages on Health Information Exchanges (HIE) as requested. Arizona Care Network (ACN) is a clinically integrated physician network and is a Dignity Health and Abrazo Health (Tenet) physician collaboration.  The Care Coordination Navigator is an employee of Dignity Health Management Services Organization.   The goal of care coordination is to assist in managing care, cost, and outcomes across the continuum of care.  The implementation of sound clinical, fiscal, and operational strategies is critical to the continued delivery of quality services. Care coordination principles provide an opportunity to balance care with cost.  The ACN care coordination program’s purpose is to promote efficiency, efficacy, and effectiveness of services for patients.   The Navigator supports the primary care team with reducing fragmentation of patient care, improves compliance and access to care, supports efforts to reduce or remove treatment barriers, and assists patients in navigating their path through the continuum of care. The navigator will be working in the community, hospital, provider offices, skilled nursing facilities, and the ACN Command Center.     PRINCIPLES DUTIES AND RESPONSIBILITIES   - Effectively works with patients, staff, health service providers, agencies, etc. from diverse backgrounds to reduce cultural and social-economic barriers between patients and institutions. - Clearly communicates the purposes and services available in the ACN Care Coordination program to patients, family members and caregivers. - As part of the Care Coordination Team, assists patients in understanding care plans and instructions and helps patients actualize health management plans and goals. - Receive patient requests for assistance and refers patient to appropriate member of ACN Care Coordination Team (PCP, Care Coordinator, Social Worker, Pharmacist) for resolution, unless Navigator can resolve on his/her own and within the scope of the position. - Coaches patients in self-management of their chronic health conditions. - Develops relationships with community resources and service providers. - Assists patient with barriers to care including but not limited to scheduling appointments, transportation needs, and access to community resources unknown or otherwise unavailable to patients without appropriate referrals. - Documents activities, plans, and results in an effective manner to maintain case files and provide reports. - Works collaboratively with the rest of the ACN Care Coordination Team, including regularly communicating feedback from patients and providers. - Fully discloses relevant training, experience and credentials, in order to help patients understand the scope of services the community resource aide is qualified to provide and refrains from any activity which could be construed as clinical in nature. - If applicable; work in collaborative matter with team in physician office embedded in, as well as collaborate with assigned QPR, RN, SW as appropriate. - Basic understanding of ALOS, re-admission rates, Gaps in Care. - Other duties as assigned by Navigator Manager.

Requirements: - Knowledge of therapeutic approaches and principles of Medical Nutrition Therapy and ability to plan, develop and deliver nutritional interventions as well as provide individualized dietary consultations. - Knowledge of and ability to assess patients and develop and implement a treatment plan. - Knowledge of education concepts and techniques; the ability to educate people about food requirement, eating habits, and therapeutic nutrition. - Knowledge of principles and practices associated with menu planning; ability to plan nutritious and varied meals for patients and staff. - Knowledge of applicable laws, rules and regulations governing dietary guidelines; ability to apply these theories to design, develop and maintain a healthy diet. - Knowledge of customer service concepts and techniques; ability to meet or exceed customer needs and expectations and provide excellent service in a direct or indirect manner.
