Under the direction of the Program Manager, The Community Health Worker is responsible for conducting outreach and education activities for identified populations. The Community Health Worker is responsible for identifying possible outreach and education locations and effective strategies to reach target population. The Community Health Worker will lead education sessions to both groups and individuals. The Community Health Worker will adhere to established procedures for outreach, education, advocacy and client referral. The focus area for The Community Health Worker will vary according to program goals. Under the direction of department leadership, the Systems Trainer is responsible for delivering effective training and support to all end users within the clinical care locations. This position ensures that training is customer focused and needs based. The Systems Trainer has delegated responsibility for problem solving and assessing processes and issues. The Systems Trainer conducts audits for quality of tasks in the electronic health record, providing feedback to leadership team. Audits for accuracy and completeness of contact center tasks. The System Trainer provides feedback and guidance to contact center staff on areas of improvement. The System Trainer determines best practices to support the continuing knowledge growth of Dignity Health Medical Foundation.
Responsibilities may include:
- Participates in building, testing and maintaining processes in the training environment.
- Collaborates in the development and maintenance of training materials, including but not limited to scripts, scenarios and training schedules.
- Delivers end user training and support, using existing training materials and procedures.
- Participates in workflow design and system build in order to gain an in depth knowledge of the system, including functionality and workflows.
- Troubleshoots and resolves issues and questions, referring more complex issues to Senior Systems Trainer or to Supervisor.
- Incorporates new system functionality in training/education plans. Identifies ways to improve education/training plans.
- Other duties as assigned.
Under indirect supervision the incumbent is responsible and accountable for all aspects of the Programs food service operation, in accordance with all program regulations from the California Department of Aging, PSA 2, the State of California Health and Safety Code. The incumbent must possess the ability to coordinate and manage all aspects of the food service operation which includes: planning menus, procurement of both food and non-food items within budget limitations, storage and inventory control procedures, preparing and directing the preparation of meals, the operation and maintenance of food service equipment, and shall supervise both paid and unpaid staff. The position reports to the Manager Operations.

About This Position
In keeping with the Marian Medical Center health care philosophy, the Cook reviews production records and assembles food supplies and equipment for daily food production.
Follows recipes and other production records to facilitate uninterrupted efficient operation of trayline, cafeteria and other service areas. Bakes, roasts, broils and steams meats, fish, vegetables and other foods. Participates in monitoring of food quality by tasting, smelling and taking temperatures. Completes set-up and placement of food items on patient trayline and other food service areas. Assists, as directed, facilitation of the flow of work in the operations of kitchen procedures and food distribution. Maintains proper care, maintenance and sanitation of work areas, equipment and supplies. Alerts supervisor to problems or needs concerning equipment, food supply sanitation, and safety. Detects and reports spoiled or unattractive food or supplies. Maintains established department policies, procedures, and objectives for quality assurance, safety and infection control.
Minimum of 1-year prior cooking experience. Minimum of 1-year prior cooking experience, preferably in a health care facility Possesses good working knowledge of basic cooking techniques. Possesses sound knowledge of food storage, usage, and sanitation principles. Must be able to set daily priorities and work independently and as a team member. Basic knowledge of institutional cooking equipment and the safe use of such. Ability to read, write and follow written and oral instructions in English.

About This Position
Prepares and serves food for patients, staff, volunteers and visitors according to established policies/procedures, governmental regulations and does this in a safe/sanitary manner. May be cross trained in several shifts throughout the day. Performs general cleaning duties such as washing utensils, pots, pans and work areas.

THE CARDIAC REHABILITATION PROGRAM at Chandler Regional Medical Center is a comprehensive program which includes education and supervised exercise to help facilitate a safe and successful recovery for those with heart disease, or those at high risk of developing heart disease. The Cardiac Rehab program is comprised of three phases each designed to meet the needs of the patients. Phase I (Inpatient), and PHII and PHIII (Outpatient). The inpatient staff work 8 hour shifts coverage 5 days a week. The outpatient staff work 10 hour shifts on M-W-F and the remaining hours are covered on Tuesday and Thursday's to meet incoming patient needs. The staff consists of RN's, Exercise Physiologists, and a registered dietitian who floats to this department to meet the cardiac diet education needs.
