Position Summary: Methodist Hospital, a member of Dignity Health, is currently looking for a Chaplain to add to their hospital staff. The Chaplain provides spiritual/religious counseling and ministry to patients and their families, demonstrating competency in spiritual support and pastoral counseling, as well as dealing with grief, the dying process and death. This individual recognizes that during illness and suffering, individuals rely on their spiritual resources to cope and find meaning. Through the ministry of presence, religious and spiritual interventions, pastoral counseling, bereavement counseling and crisis intervention the Chaplain is empowered to heal, sustain, guide, and reconcile patients and their families. This individual also provides a supportive presence to staff, including physicians, and makes referrals when necessary. The ideal candidate will have a working knowledge of systems and organizational dynamics with the ability to align personal and departmental goals with the organizational goals. The ability to articulate the role of chaplains and spiritual care services they offer to various interest groups in various organizational settings is essential. Minimum Qualifications:
- At least 1+ year Chaplaincy experience in a hospital or healthcare setting and at least 4+ units of clinical pastoral education
- Candidate must be APC or NACC certified or Board Eligible

About This Position

This position performs and is responsible for a variety of nursing functions that include; assessment, identification of needs, planning, implementing appropriate interventions, documentation, evaluation, explanation of procedures, checking for and or obtaining consents, obtaining vitals, observing cardiac monitors, operating suction, oxygen, and power injectors and maintaining patients' comfort during radiographic procedures. These procedures include but are not limited to percutaneous biopsy, myelography, bronchoscopy, stress echocardiograms, transesophageal echocardiograms, pediatric voiding cystourethrograms, CTs, MRIs, Dobutamine echos and any procedure requiring sedation and analgesia or contrast injection. The position is also responsible for nursing administrative duties such as invasive procedure patient follow-up, patient education including discharge instruction, radiology staff in-service education, and assistance in procedure manual upkeep. The radiology nurse will assist in coordinating the patient's schedule to insure that all procedures are performed in a safe and efficient manner. This position also addresses the psychosocial needs of the patient specifically in regards population specifics and acts as educator and resource person to patients and their families. The position may also provide nursing back up in the Special Procedures and Cardiovascular labs. This position takes direction from radiologists, cardiologists, and other physicians as well as the Radiology Manager.
EXPERIENCE:

Position Summary:
The Hierarchical Condition Category (HCC) Quality Program was developed by CMS to promote quality care for Medicare Advantage members. By focusing on comprehensive documentation to identify, evaluate and assess chronic conditions at the appropriate specificity, patient medical needs are met at the highest level. The QCDI Coding Specialist's primary focus will be to facilitate and ensure the comprehensive capture of billing data for the purpose of accurately reporting HCC's; participating in the reconciliation of patient medical and billing records; identifying reimbursement/quality of care opportunities. Additionally this role will provide provider education and review trends of assigned provider or medical groups.
Responsibilities may include:
- Performs comprehensive chart reviews to ensure documentation required to facilitate the reporting of HCC diagnoses to payer.
- Identifies claims correction opportunities and submits to appropriate personnel for processing.
- Demonstrates analytical and problem-solving ability regarding review of submitted diagnosis codes versus services reflected in the documentation in the patient's chart note.
- Follows department policies and guidelines on appropriate documentation to billing codes, abstracting information from chart notes based on performance program measures.
- Partners with QCDI Nurse, as necessary, to identify trends and gaps for creating better process efficiency and provider performance.
- Assists in the development and reporting of HCC and Pay for Performance Metrics.
- Adheres to deadlines and insure reports are completed and distributed to all concerned parties.
- Provides structured and ad hoc training/education to staff and providers.
- Performs necessary analysis of data for the purpose of identifying trends and making suggestions for change to process. Develop action plans based on analysis.
- Works collaboratively with Optum, Revenue Cycle Staff, Business Intelligence-Decision Support Staff, Clinical Informaticist and Dignity Health staff associated with HCC initiative.

Position Summary:
The Quality Clinical Documentation Improvement (QCDI) RN is responsible for performing a comprehensive review of ambulatory medical records to confirm documentation compliance with the Centers for Medicare and Medicaid Services (CMS) programs and other quality initiative programs Dignity health Medical Foundation participates in. The primary focus of the QCDI RN position is to review provider documentation, provide education to providers, and review trends of assigned providers or medical groups. Additionally, the QCDI RN will provide guidance to Reimbursement staff in their day-to-day chart reviews.
Responsibilities include:
- Reviews high volume of EMR medical records to determine if specific conditions were assessed and documented appropriately; Follows department policies and guidelines on appropriate documentation to billing codes, abstracting information from chart notes based on performance program measures.
- Identifies claims correction opportunities and submit to appropriate personnel for processing in a timely manner within pre-set acceptable accuracy levels. Tracks trends for each physician to identify potential training areas.
- Maintains accurate, timely tracking of physician documentation performance using established tools within the department; Alerts management when low performance trends occur.
- Provides chart review guidance and education to team regarding appropriate chart review methodology when necessary; Performs secondary chart reviews when applicable to clarify compliance to coding guidelines.
- Uses knowledge of clinical processes and medical acumen to answer team questions regarding acceptable documentation; Recommends provider, staff, and clinic training based on trends.
- Provides assistance to team coordinator with annual spring training and annual one on one sessions, distribution of reports, reconciliation of quarterly and year end statistics, and other provider related activities.
- Participates in training and orientation of staff.
- Follows up with physicians and providers about missing or incomplete chart documentation via tasking and/or email. Provides guidance and education to assigned physicians for improved documentation performance.
- Keeps manager informed of concerns in workflow, policy, or processes as it related to the organization's programs and interactions with physicians and clinic staff. Works collaboratively with team members to sustain a standard approach to chart reviews, accurate reporting, and performance tracking.

Under the direction of the Department Manager, the Senior Office Coordinator is responsible for Daily code capture of the professional fee of follow ups, new patients in IMPAC, and front office procedures. Front office duties include maintaining the department schedule while keeping flexibility and a consistent standard of quality in mind. In addtion demonstrates initiative and creativity in recommending changes in front office policies and procedures to the Department Manager.

Position Summary:
Manages the daily operation of all services of the Respiratory Therapy Department. Coordinates the activities of the assigned departments with other departments within the facility. Recruits, hires and retains qualified staff.
Core Duties:
- Manages the daily operations of the Respiratory Care Department to ensure compliance to all standards of practice and licensing/regulatory requirements.
