The Rehabilitation Admissions Coordinator performs activities and duties related to pre-admission evaluation and admission of patients to the inpatient rehabilitation unit. The goal of the coordinator in this role is to identify patients who meet program criteria and who will benefit from an treatment in an acute rehabilitation program. Included in this role, is the ability to review medical records to gather information to complete the pre-admission screening tool to review the appropriateness of the patient for admission, with MD collaboration and case management to facilitate transfer in a timely manner; ensures that benefit authorization are completed as required; determines with nursing/others, bed and team assignments; builds relationships with external referral sources; gathers/tracks data to be utilized in planning; ensures compliance with regulatory/accreditation organization.

Job Summary:
Our Interventional Radiology Department is seeking a Staff RN to join our team! Assess patient/family health problems and resources, taking a leadership role in the development, implementation and outcomes evaluation of a plan for nursing care. Provides professional nursing care and interventions to acutely ill adult patient populations.
SUMMARY - Serves as a member on Neuro-Vascular team responsible for coordination of care for stroke patients from admission to discharge, and post discharge chronic care in the outpatient setting. Procedures: TCD and loop recorder-LINQ. Also serves as member of the Stroke Team responsible for the coordination of care for the stroke patient from admission to discharge, including those that enter through the TeleStroke network. The Stroke/TeleStroke Program coordinator acts as a resource to staff, the multi-disciplinary team and community caring for stroke and telestroke patients. The Stroke/Telestroke Program Coordinator is responsible for managing the logistics of the TeleStroke network. The coordinator demonstrates knowledge and skill in telemedicine, including hardware systems and communications. The coordinator assists in the development of policies and procedures, order sets and pathways to improve quality of care and program development for both the Barrow Stroke Program and the Barrow TeleStroke programs.

Circulating nurses work in hospitals and assist with operations and other surgical procedures by serving as an intermediary between the staff working in the operating room and the rest of the hospital. Circulating nurses have a variety of duties relating to this function. These duties include but are not limited to patient assessment, equipment and supplies, assisting the surgical team during procedure, and specimen processing. SHIFT CALL IS REQUIRED!
