Performs the nursing process for patients in more complex situations with assistance and supervision.  Assists with managing the patient care administered by other member of the nursing staff.  Assumes the responsibilities of Primary Nurse and Team Leader. Educational Requirements:              - SBAR communication class/ therapeutic education class - Demonstrated competency in 12 lead EKG  License/Registration/Certification: - Current and active CA RN license. - Current BLS certification. - Unit specific certifications and qualifications. - Current ACLS and PALS certification - NIH Stroke certification - Annual Dominican Stroke certification education - CCRN, CAPD certifications desired - Three year minimum ICU experience (ER and/or CIU experience will be considered on an individual basis). Position Summary: (Unit Specific)RAPID RESPONSE CALLS: - Primarily responsible for rapidly responding to the bedside of a patient who has been identified as having acute changes in his/her condition and/or showing signs and symptoms of stroke or shock. The Rapid Response RN (RRRN) functions as a resource RN for the primary RN caring for the patient.  In this role, the RRRN assists in assessing the patient, consulting with MD and initiating treatments as ordered by the MD. The RRRN initiates RRT standardized protocols. The RRRN monitors the patient as needed or assists in transferring the patient to a higher level of care.   He/She may assume care of the patient until a bed and or RN is available to take over the care of the patient.  The RRRN follows up with each rapid response call over the next 24 hours or longer if needed.  - Stroke/TIA:  responds to any potential stroke patients on the floors, assess the patient according to the Acute Stroke SBAR, notify the neurologist on call for any suspected strokes, and initiate Code Stroke Alert if instructed to do so by the neurologist. They will assist in getting the patient to the correct level of care as indicated by the neurologist.  RRT will provide stroke teaching to patients with diagnosis of stroke/TIA for discharge. - Rapid response calls may also include dealing with code status changes as well as dealing with difficult families or patients. The RRRN must be able to use therapeutic communications skills and be able to assess and handle emotional distress in patients and/or families.  Makes referrals to Palliative Care as needed. PATIENT FLOW DUTIES/ ADMIT RN DUTIES: - RRRN will complete the admission process for ED patients and direct admits as time permits in the hierarchy of RRT priorities and triage. The RRRN will attend the 0830and 0100 bed huddle.  The RRRN will collaborate with the Nursing Supervisor to promote flow and patient placement. The RRRN may confer with case management as well. This knowledge combined with excellent critical nursing judgment allows her/him to assist in bed placement issues.  During times of bed crisis, the RRRN may assist in opening and closing 2 Central. - When the ED is impacted, the RRRN can assist with patient flow by transferring and admitting patients to the floor. The RRRN understands the different levels of care that each unit provides and uses his/her nursing judgment to assist ED staff in assuring that patients are admitted to the appropriate floor.   - The RRRN provides support and education to the RN’s on the floor by collaborating and sharing his/her skills regarding patient care in a non-judgmental and non-critical manner.  - By working closely with the primary RN at the bedside, knowledge and skills are shared and the primary RN becomes more comfortable and knowledgeable in assessment, planning, implementation and evaluation of patients who are acutely deteriorating.  The RRRN can offer resource and support to the new grad, decreasing the feelings of fear and isolation. In addition to bedside teaching, the RRRN is available for formal lectures and workshops as needed.  As an experienced communicator, the RRRN will play an active role in increasing the skill of the bedside RN in SBAR communication. Promoting his/her role and having a open and trusting relationship with staff throughout the hospital is an important part of the RRRN position and ensures that the RRRN is called whenever needed by the floor RN’s. PROCEDURES: - In order to reduce length of stay and assure that necessary procedures are done in the most timely manner, the RRRN is available to assist with bedside procedures, cardiopulmonary procedures; ie: TEE’s, cardioversions and Cat Scan’s and MRI’s with procedural sedation.  She/He can assist with transfers to and from the permanent and portable MRI trailer. - RRT can assist floor RN’s with difficult IV starts when IV RN is not available. - Assist L&D in preparing patients for crash C-Sections - Nuclear Med: Administration of CCK and Lasix injections. MEAL BREAK RELIEF: - Can provide meal break relief when no other staff is available. Position Responsibilities:  - The RN is responsible for supporting efforts to improve clinical and service excellence of patient care provided. RN s must be able to identify and apply care skills appropriate to the specific patient age groups served. The RN will participate in the achievement of service targets and process goals. ·        Coordinates the clinical operations for the Rapid Response Team program. Position Summary: The Clinical Support Specialist I performs clinical applications support duties with the clinicians and staff providing patient care in the ambulatory clinic setting. These responsibilities include the initial computer-based training and orientation of physicians and staff as well as the on-going computer skills improvement and enhanced competency training associated with the clinical applications. The CSS I will participate in workflow design, development, and re-engineering as upgrades, enhancement requests, and new regulations are presented. The CSS I will participate as assigned on project implementation teams; workflow documentation initiatives; and clinical enhancement and optimization efforts. This position has primary responsibility for initial problem triage and assignment, first call issue resolution, critical problem solving, and collaborating/escalation with other IT support teams. • Maintains working relationships with clinicians, staff, and clinic leadership at assigned locations to ensure consistent delivery of all IT support services. • Acts as assigned resource to the various project teams assisting in the design and development of the continually improving ambulatory applications suite of solutions. • Performs the delivery of services and escalates any conflicts that may arise between matrix teams, project teams, team members, facilities, and ambulatory clinics. • Utilizes the knowledge and skills of clinical practice to recommend clinical functions that are suitable for computer applications and ensure the clinical applications are consistent with professional standards of clinical practice. • Contributes as assigned in ambulatory governance meetings, department meetings, and staff meetings. • Supports the Physician Integration functional planning process; including needs assessment, regulatory requirements, and new clinical service requirements involving key stakeholders and utilizing structured analysis methods for planning. • Delivers consistent clinical support services both within and outside the defined Service Level Agreements. • Assists in the development of process and operational redesign recommendations leading to a solution and/or functional system requirements. • Provides clinical support in all phases of a project plan including system testing, conversion planning, and training material development. • Communicates functional priorities, design techniques, issues, and issue resolution to leadership. • With others, promotes the continuity of the ambulatory computing environment and the application functionality in accordance with the intended design as deployed from clinic to clinic, market to market. • Monitors application use, system performance, hardware reliability, and software stability. Captures, defines, and prioritizes pre- and post-implementation issues. Reports and participates in the problem solving of issues that impact local and enterprise performance. • Communicates effectively, and contributes to positive interdepartmental relationships; particularly between technical and clinical staff. • Takes immediate action to meet customer/user requests or needs. • Builds relationships with peers and staff. Ensures accountability relative to timelines, work, and product, in support of reporting metrics and protocol compliance. • Uses critical thinking skills and job knowledge to resolve job-related issues. • Maintains confidentiality of all patient and Dignity Health information, communications, and documents. • Participates in the orientation and training of new physicians and staff. Provides on going assistance to ensure technical skill sets of physicians and staff maintain a standard minimum competency. • Provides assistance relative to technical skill sets for staff and incorporates them into the associated training. • Constantly evaluates training materials ensuring content is in alignment with application versions and appropriately provides end users with appropriate skills to be successful. • Demonstrates extraordinary customer relations, quality service, and dedication to providing the highest quality of service to our clinicians, staff, and patients. • Supports the efficient and effective operations of Ambulatory Clinical Information Systems, the promotion of effective user adoption, and success in deployment of these systems. • Ensures a professional and responsive atmosphere by greeting patients, visitors, staff, and physicians promptly and courteously. Refers to each individual by name when appropriate. • Upholds the mission, vision, and values of Dignity Health, as well as our sponsoring congregations. Minimum Qualifications: • 2+ years of experience providing system support to clinical staff, or working in a clinical setting with daily use of an electronic health record. • 2+ years of experience working with clinical workflows within an electronic health record. • 2+ years of proven knowledge of clinical workflows and documented ability to provide excellent support to clinical staff. • Knowledge of ambulatory/outpatient care environments and workflow processes. • Strong oral, written, and presentation skills. • Ability to work in a complex and matrixed team environment; experience working in a team-oriented, collaborative environment. • Ability to handle multiple tasks, and to set priorities, schedule, and meet deadlines. • Computer literacy (use of computer technology to accomplish tasks) and information literacy (ability to recognize, locate, evaluate, and use information appropriately). • Ability to effectively train others in the use of computers and related technologies. • Strong communication skills, ability to translate most concepts to naïve computer users and various levels of personnel from executives to end-users. • Ability to create effective interpersonal relationships.  Preferred Qualifications: - MA, LPN, LVN, RN license or other clinical degree desired. - Clinical licensure is desired, but a combination of experience and education can be considered in lieu of a clinical licensure. - Experience with Allscripts or Cerner preferred.
