    The purpose of Dignity Health Management Services Organization (Dignity Health MSO) is to build a system-wide integrated physician-centric, full-service management service organization structure. We offer a menu of management and business services that will leverage economies of scale across provider types and geographies and will lead the effort in developing Dignity Health’s Medicaid population health care management pathways.     The Arizona Care Network (ACN) is a clinically integrated physician network is a Dignity Health and Abrazo Health physician collaboration.  The Quality Data Abstractor is an employee of Dignity Health Management Services Organization (Dignity Health MSO).


    The Quality Data Abstractor supports the Clinical Performance Department by locating specified clinical data for abstraction from various forms of medical records through intensive medical record review. This position abstracts clinical information from paper or electronic medical record and populates data collection tool to support compliance with quality measures.




    • Gains clear understanding of criteria for compliance with ACN clinical quality measures (numerator and denominator).
    • Enters patient demographics into appropriate data collection tools.
    • From measures identified in data collection tool, reviews chart for applicable documentation to support quality measures. Gathers data from patient charts and record results via data collection tool.
    • Ensures a high degree of accuracy of information entered.
    • Works with practice physicians and clinical staff to set abstraction appointments, pulls charts according to the protocol and arranges for all necessary tools to conduct abstractions.
    • Collaborates with Data Analytics team to ensure accurate reporting of abstracted information from quality measures.
    • Represents ACN and members interest in supporting physicians to improve clinical quality documentation.
    • Provides accurate and prompt post-assessment findings to the Clinical Performance Department for review and follow-up.
    • Identifies opportunities for the Clinical Performance Representatives to assist providers/staff with documentation, work flow and/or process improvements to ensure clinical quality.
    • Aggregates summary data, tracks statistics, conducts analysis and completes standard reports.
    • Strictly adheres to confidentiality and established Protected Health Information guidelines required.
    • Other duties as assigned.


    A) EXPERIENCE: Minimum two (2) years experience charting in medical records and/or chart data abstracting experience, preferred. HEDIS/GPRO experience strongly desired.

    B) EDUCATION AND TRAINING: High school diploma or equivalent required.


    D) OTHER REQUIREMENTS: Computer proficiency as well as knowledge of database entry.  Strong organizational skills, required. Knowledge of general office procedures and Microsoft Office applications. Excellent written and oral communication and interpersonal skills, required.
