Arizona Care Network (ACN) is a clinically integrated physician network and is a Dignity Health and Abrazo Health (Tenet) physician collaboration. The Social Worker is an employee ofDignity Health Managed Services Organization, a physician support organization owned by Dignity Health.
The Social Worker will work at the height of their professional licensure to participate in the implementation of integrated health services and behavioral health-based care coordination services. The Social Worker is a key liaison between the primary care team and behavioral health team, and will work very closely with the medical providers, behavioral health counselors, care coordinators, and patients to meet the patient’s multiple clinical and non-clinical health needs through a strength-based model. The Social Worker will be working in the community, medical offices, skilled nursing facilities, hospitals and the ACN Command Center.
What We’re Looking For
• Masters Degree in Social Work
• Minimum 3 years experience performing social work; preferably in ambulatory setting/ bilingual a plus
• LMSW or LCSW from the AZ Board of Behavioral Health
• Clinical training in a medical or clinical environment. Chronic disease management training. Knowledge of evidence-based clinical practice guidelines.
• Proficient in electronic clinical documentation.
