    Responsibilities

    The Supervisor of Care Coordination is responsible for the oversight of day to day departmental operations including , but not limited to: clinical resource management, discharge planning activities, patient advocacy, clinical social work and best practice in medical necessity determination and concurrent review. This position will supervise the department in an efficient manner to ensure timely and compliant care coordination, discharge planning, utilization review and social work interventions resulting in quality patient care. The Supervisor may assign work tasks, realign tasks within the department and take action to resolve minor disputes and productivity issues. This position may be assigned other duties within scope of practice, including, but not limited to : scheduling, conducting audits, performance improvement activities, assistance in  staff  evaluations or assuming a case load based on departmental needs. The positions focus will be on  optimizing staff productivity, collaboration and compliance . The Supervisor provides  of support and coaching to staff members as indicated. This position will report to and collaborate with the Manager or Director regarding identified issues outside the Supervisors scope of authority.The Supervisor  guides department activities according to the organizational objectives, hospital policies, standards of practice and Federal and State Regulations.

    Qualifications

       Experience Required:

    • Comprehensive knowledge of care management, clinical social work and discharge planning.
    • Minimum of 3 years in a hospital based Clinical Social Work role.
    • Working knowledge of Medicare review process and regulations.
    • Effective communication skills
    • Knowledge of Conditions of Participation and Joint Commission standards.

    Education Required:

    • Masters Degree in Social Work from a university or college accredited by the council of Social Work Education.

    Special Skills Required:

    • Excellent customer service and presentation skills are a must
    • Strong interpersonal and written communication skills are essential
    • Demonstrated ability to apply analytical and problem solving skills
    • Demonstrated ability to manage multiple tasks or projects effectively
    • Ability to work independently as needed with a high degree of detail orientation
    • Capable of planning and organizing projects with short notice
    • Ability to work efficiently in a fast-paced environment with changing priorities

    Licensure/Certification Required:

    • Current State License for Social Work (LCSW)
