    The purpose of Dignity Health Management Services Organization (Dignity Health MSO) is to build a system-wide integrated physician-centric, full-service management service organization structure. We offer a menu of management and business services that will leverage economies of scale across provider types and geographies and will lead the effort in developing Dignity Health’s Medicaid population health care management pathways. Dignity Health MSO is dedicated to providing quality managed care administrative and clinical services to medical groups, hospitals, health plans and employers with a business objective to excel in coordinating patient care in a manner that supports containing costs while continually improving quality of care and levels of service.     The Community/Population Health Program Manager is responsible for supporting community and population health initiatives that impact and improve the lives of the vulnerable population. Manages the development and operations of community health improvement programs and departmental functions.  Develops and participates in strategic, collaborative partnerships with hospital and community stakeholders to achieve goals.  Supports assessment, planning, evaluation and reporting of community health program initiatives focused on improving the lives of the value-based patient populations, specifically focused on the Medicaid/uninsured populations.

    7. Must be able to travel to locations both in and out of town which may require overnight stays on occasion.



    1. Responsible for meeting all of the DHMSO Job Standards described below.
    2. Assists in the development and facilitation of community/population health initiatives aimed at advancing and aligning population health, physician integration, and community health throughout the system.
    3. Supports the identification and integration of business needs and technology solutions, helping to create and implement approaches that meet community health and population health objectives.
    4. Supports the planning, design, implementation, training and reporting of the Coordinated Community Network Initiative.
    5. Works collaboratively with other hospital and population health management departments (e.g., community health, care coordination, nursing, analytics and philanthropy) and with external community organizations and agencies to address identified priority needs and improve health and quality of life.
    6. Provides operational support for the Community Health department, including deployment and management of staff and other resources.
    7. Incumbents in this position shall comply with all applicable laws and rules including the Dignity Health Standards of Conduct and Dignity Health policies and procedures that apply to his/her job responsibilities, seek guidance when in doubt, promptly report any potential or suspected violation of Dignity Health Standards of Conduct, Dignity Health policy or procedure or applicable laws/regulations and when requested, assist Dignity Health personnel in investigating all allegations of violation. When called upon, the incumbent shall assist in risk evaluation and work collaboratively with facility/entity leaders to implement, monitor and mitigate risk corrective plans outlined by the Compliance program



    1. Three or more years’ experience working in a community or public health role.
    2. Bachelor’s degree from an accredited college in health administration, public health, or a comparable combination of education and experience is required.
    3. Experience working in a healthcare, public health, or other related business environment coordinating projects.
    4. Experience working with project management tools (Microsoft Project, Microsoft SharePoint, Microsoft Visio) preferred.
    5. Must have valid California drivers’ license and clean DMV record.
