The purpose of Dignity Health Management Services Organization (Dignity Health MSO) is to build a system-wide integrated physician-centric, full-service management service organization structure. We offer a menu of management and business services that will leverage economies of scale across provider types and geographies and will lead the effort in developing Dignity Health’s Medicaid population health care management pathways. The Community/Population Health Program Manager is responsible for supporting community and population health initiatives that impact and improve the lives of the vulnerable population. Manages the development and operations of community health improvement programs and departmental functions. Develops and participates in strategic, collaborative partnerships with hospital and community stakeholders to achieve goals. Supports assessment, planning, evaluation and reporting of community health program initiatives focused on improving the lives of the value-based patient populations, specifically focused on the Medicaid/uninsured populations.
