About this opportunity:
Under the direction of the Clinical Coordinator and Program Director, the Case Manager (RN) will perform the duties of patient care. This person is responsible for treatment, safety, patient care coordination, quality management, and patient, professional, and community education. This RN is also responsible for coordinating clinical services and assuring consistent quality care for patients in the program, working within the framework of the State Nurse Practice Act and following all Healogics and Glendale Memorial Hospital and Health Center's policies and procedures.
Wound care experience, minimum 1 year in a Home Health agency, SNF or in an acute care setting as a wound care nurse required. Graduate of a state approved RN program with RN licensure. BSN preferred or matriculating in a national or state approved BSN program. Current California license as a Registered Nurse. BLS required. Wound Care certification preferred.
