This position provides ongoing intensive group psychotherapy and occasional individual and family sessions as needed in partial hospitalization and intensive outpatient programs. Works closely with multidisciplinary treatment team in the development of treatment plans and discharge planning. Makes referrals for financial or other identified resource needs and facilitates continuity of care for post-discharge and engagement of appropriate agencies or community resources. Maintains timely and thorough documentation on patient care activities. May also participate in managed care reviews for medical necessity authorizations. Works with an adult population with mental health diagnoses and some patients with dual diagnosis. Ability to collaborate and communicate with members of the medical staff including psychiatrists. May occasionally float to inpatient department to provide assistance.
This is a Monday through Friday program and is closed on weekends and holidays. Environment includes a warm and inviting office space and comfortable program area for participants
The work schedule for this benefited part-time position would be Monday, Wednesday and Fridays only.
