The Nurse Navigator represents Dignity Health with our local, regional, and international patients. The Nurse Navigator ensure the right services meet the patient needs. Complex problem solving, strong analytical skills and the ability to deal with ambiguity are required. A solutions oriented patient advocate with a passion to help others is desired. This role is responsible to identify and initiate referrals for appropriate care and programs. The incumbent is responsible for communicating with leadership and stakeholders related to program and patient care coordination.
The Nurse Navigator ensures interdisciplinary, patient-focused, well-coordinated system of care of patients - collaboratively assessing, planning, and facilitating the timely coordination of quality care while eliminating barriers and improving the patient's experience. The Patient Nurse Navigator is expected to work closely with the staff and physicians to create: 1. Seamless direct patient coordination, guidance, education, referral, and follow up to ensure an integrated and diversified delivery of care. 2. Serves as the patient's personal health liaison and is a primary resource for questions, becomes familiar with patients treatment plan and helps to coordinate multidisciplinary approach to care and navigation of entire treatment plan. 3. Provides on-going patient follow-up, calls and visits during on-going care and hospitalizations as well as emotional/psychosocial support to the patient and the family. 4. Offers patient appropriate bundles, affordable packaged pricing, and works with finance to provide/address the right mix of services to meet patients' care needs. 5. Assist in the development of bundles, access protocols, and processes to build the program and manage ongoing operations.
Five (5) years recent acute care experience
Completion of accredited RN program and Clinical Experience
Strong verbal and written communication skills
Current Arizona RN License
BLS (American Heart Association)
Strong Business Acumen
2 years call center or tele-health/telephone nurse experience
Experience referring to or arranging clinical and community resources
Insurance/benefits coordination experience preferred
Program management experience is desirable
