    The Center for Transitional NeuroRehabilitation (CTN) offers intensive, day-long treatment for older adolescents and adults with brain injuries. The program uses a holistic approach to treat cognitive, language, physical, emotional, psychological, and vocational needs. The CTN emphasizes independence in the home and community and productivity when patients return to work or school. For family members, the CTN provides emotional support and education about the patient's strengths and limitations and how to assist the patient in functioning in the home. The CTN offers four rehabilitation-oriented programs: the Home Independence Program, the Work Re-Entry Program, the School Re-Entry Program, and the Transitional Program.


    Join us NOW as our CTN Department seeks a Licensed Physical Therapist to join our team!  Provides patient care including evaluating patient for treatment, providing and directing administration of the Physical Therapy care plan upon Physician referral. Consults with other health care disciplines to co-ordinate therapeutic programs and discharge planning for individual patient. Instructs patients and care givers in home programs for exercises and functional activities.Assists with patient program management, while also delivering patient services. Serves as a resource by sharing expertise and knowledge and assisting with staff professional development. Performs patient clinical care.


    Located in Phoenix, AZ, this position is critical to the success of St. Joseph's Hospital Medical Center and requires the full understanding and active participation in fulfilling the Mission of Dignity Health.





    Graduate of an accredited Physical Therapy Program
    B.A./ B.S./ or DPT - Graduate of Physical Therapy Program 
    Arizona Physical Therapy Licensure
    I'm a Physical Therapist
