    Position Summary

    The Stroke Program Coordinator is responsible for assessing, planning, developing, coordinating and implementing patient care strategies and objectives for stroke patient related activities. The focus of the program is patient centered disease specific care, through program management; thus, maintaining a multi-disciplinary approach/collaboration to patient centered care.

    • Sustaining and maintaining adherence to the Joint Commission (TJC) Disease Specific Certification for Primary Stroke Centers, Clinical Practice Guidelines, Standards of Care, Elements of Performance, Brain Attack Coalition (BAC) and JC National Patient Safety goals.  
    • Additional responsibilities include, acting as a liaison between patients and other healthcare providers, coordinate functions as a resource for patient, family and nursing staff education, maintain and establish patient support programs and materials, promotes community awareness and education, incorporates high quality patient care, education and community service into daily practice supporting the mission of the Dignity Health system, facilitate plans for patient care and plan for nursing care across the continuum by rounding daily on all stroke patients and “Code Stroke Alert” patients, respond to Code Stroke Alerts to assess/facilitate high quality patient care and ascertain protocols are followed.
    • The Stroke Program Coordinator will maintain competency as a registered nurse, attend reputable stroke care conferences, thus contributing to the standards of care for stroke patients by educating the Stroke Team on findings.
    • The coordinator will identify safety concerns and institute measures to provide a safe environment and provide appropriate assessment, intervention, and evaluation to patients with consideration of developmental milestones in the adult and geriatric population.
    • Provides nursing care consistent with the Nursing Standards of Care, the Plan for Provision of Care, and Professional Organizational Standards.



    Minimum Qualifications

    • BLS required; ACLS may be required based on specialty unit
    • RN Required
    • Current licensure as a registered nurse in the state of California
    • Demonstrated teaching skills, leadership skills, skills in critical thinking, and reading are essential
    • Current clinical experience in an acute care hospital


     Preferred Qualifications

    • BSN preferred
    • Knowledge of program plan development, adult learning skills, and familiarity with regulatory and accrediting agency standards preferred
