Mercy Medical Center Redding offers comprehensive health care to nearly 300,000 residents in a six-county region.  Mercy Medical Center Redding is an award-winning 267-bed regional medical center providing inpatient and outpatient services as well as specialized cardiovascular care, stroke care, orthopedics, neurological surgery, comprehensive cancer care, maternity care, and da Vinci robotic surgery.  It is one of only two Level II trauma centers north of Sacramento and houses the only Community Level III neonatal ICU in Shasta County.  In addition, the hospital’s network of care includes Mercy Home Health and Hospice, Golden Umbrella Adult Day Health Care, and Shasta Senior Nutrition Program.  Mercy Medical Center Redding is a member of the Dignity Health system of health care providers and are a sister facility to St. Elizabeth Community Hospital in Red Bluff and Mercy Medical Center Mt. Responsibilities

 

Position Summary:

 

Under direct supervision the incumbent is responsible for assessing, planning, facilitating and advocating for options and services through a continuum of care from point of contact through discharge on assigned patients. The incumbent performs this role in such a manner as to meet the individuals health needs while promoting quality and cost effective outcomes. The position's emphasis will be on care coordination, communication and collaboration with nursing, physicians, departments within the medical center, insurers and post acute service providers to pace the care toward optimal outcomes within the appropriate level of care. 

 

The incumbent will advocate for the patient and family by identifying and valuing patient choice, spiritual needs, cultural, language and socio-economic barriers to care transitions. In addition, the incumbent will protect confidentiality while striving to achieve high levels of patient satisfaction. The incumbent consistently conducts the utilization review process and the discharge planning process as required by hospital policies, standards of practice and Federal and State regulations. The position reports to the Manager Care Coordination.

 

 

Qualifications

Minimum Qualifications:

 

  • Associate degree in Nursing from an accredited school of nursing required
  • Current California RN license required
  • Three (3) years of experience in nursing within an acute care hospital setting and prior care coordination experience in a clinical or insurance setting required (may waive based on operational needs)
  • Master's of Science in Nursing with a concentration in Case Management may serve as a substitute for the experience requirement
  • Excellent customer service and presentation skills required
  • Strong interpersonal and written communication skills required
  • Demonstrated abilities to apply analytical and problem-solving skills and to manage multiple tasks or projects effectively required
  • Abilities to work independently as needed with a high degree detail orientation and to work efficiently in a fast-paced, ever changing environment required

 

Preferred Qualifications:

 

  • Bachelor of Science in Nursing degree or equivalent experience preferred
  • CCM certification preferred
