    In keeping with the Dignity Health core values, the Manager of Care Coordination is responsible for the oversight of Care Coordination and Social Services, including, but not limited to: clinical resource management, discharge planning activities, patient advocacy, clinical social work and best practice in medical necessity determination and concurrent review. This position will oversee the department in an efficient manner to ensure timely and compliant care coordination, discharge planning, utilization review and social work interventions resulting in quality patient care.

    • Valid California Registered Nursing License
    • Current CPR Certification
    • Minimum of three (3) years in a Care Coordination or Case Manager role
    • Minimum of three (3) years team lead, charge, or supervisory experience in an acute hospital setting preferred
    • BSN degree or experience equivalent
    • Case Management certification preferred
    • Master's degree or equivalent is preferred
