SUMMARY - The Patient Logistics Coordinator (PLC) functions as a facilitator of throughput by expediting appropriate placement of patients within Dignity Health Facilities (St. Joseph's Hospital and Medical Center and St. Josephs Westgate). This position incumbent's effective communication and interpersonal skills to assist with the process of efficiently and effectively coordinating movement of patients throughout the designated Dignity Health facilities. In addition to facilitating and managing internal facility throughput; the PLC functions as the liaison for inter-facility transfers for direct admissions when patient requests are placed to transfer patients to Dignity Health St. Joseph's Hospital and Medical Center and St. Josephs Westgate. The PLC expedites incoming calls, identifies level of hospital service needed, contacts physician or on-call physician, and connects sending physician to accepting/admitting physician. The PLC facilitates inpatient to inpatient and ED to inpatient transfers. The PLC follows internal guidelines to expedite the above functions.



High School Diploma or GED

One year previous experience in a health care setting

Basic Typing Skills Basic Computer Skills with knowledge of Word Application



Medical Assistant Diploma

One year previous year experience in a call center or front office scheduling

Invision Cerner TeleTracking Excel

Previous experience with high volume call setting and multitasking
