The Service Coordinator - Social Worker, under the supervision of the Director of Nursing and Administrator, performs social service functions, within the pediatric/sub acute unit. Serves as member of the interdisciplinary team, liaison to advocacy agencies, and support resource to residents, families and peers. Assures compliance with the state and federal regulatory and accrediting agencies such as California Department of Public Health, Title 22, and OBRA. This position requires the full understanding and active participation in fulfilling the Mission of Community Hospital of San Bernardino. It is expected that the employee will demonstrate behavior consistent with the Core Values. The employee shall support Community Hospital of San Bernardino strategic plan and the goals and direction of the quality improvement, process, and activities.
