To assist in meeting the psychosocial needs of patients and their families and to enable patients to utilize medical care to achieve their optimal level of health. Is responsible for the coordination of community health support services and facilitates patients achieving their highest level of functioning. Is the patient’s advocate in effectively utilizing Health Center services and assists patients in resolving problems with Health Center systems or personnel. Five years experience in Medical Social Work Setting
Bachelors degree in Social Work
LICENSED CLINICAL SOCIAL WORKR (LIC CSW)
Ability to read and speak English clearly and be understood.
Complete annual safety training. Complete and maintain department specific training
