Responsibilities

To assist in meeting the psychosocial needs of patients and their families and to enable patients to utilize medical care to achieve their optimal level of health. Is responsible for the coordination of community health support services and facilitates patients achieving their highest level of functioning. Is the patient’s advocate in effectively utilizing Health Center services and assists patients in resolving problems with Health Center systems or  personnel. Applies Dignity core values to the performance of every job duty. These values include dignity, collaboration, justice, stewardship and excellence.

Qualifications

Experience

Five years experience in Medical Social Work Setting

Education

Bachelors degree in Social Work

Licensure

LICENSED CLINICAL SOCIAL WORKR (LIC CSW)

Special Skills

Ability to read and speak English clearly and be understood.

Training

Complete annual safety training. Complete and maintain department specific training
