The Home Health Physical Therapist will work together with a multi-disciplinary team to provide care, identify functional goals and outcomes, and increase independence for our patients and their families. Home physical therapists specialize in aiding patients who have limitations in function by examining and evaluating the patient to develop and execute a plan of care in collaboration with the physician.
Home health physical therapists will have the ability to work autonomously, and participate in care coordination with the treatment team, physicians, and referral sources. The physical therapist will work with patients, their families and the care team to achieve recovery and to re-engage the patient with their community.
Must have comprehensive knowledge of the principles, techniques, and equipment used in the application of physical therapy.
Graduate from an accredited Physical Therapy school.
Must be able to prepare and carry out appropriate therapy care plans.
Must be able to manage his/her time effectively.
Must have own transportation and be able and willing to travel to the patient's place of residence.
LIC PT = Licensed Physical Therapist in State of NV.
