Responsibilities

Responsible for overseeing and participating in the facility's sepsis program and is accountable for assessing and coordinating the management of potential sepsis patients or those who have been diagnosed with sepsis. The Sepsis Coordinator serves as a role model and works collaboratively with the medical and hospital staff to provide multidisciplinary care to sepsis patients which includes the following: researches and drives implementation of evidence-based practices as related to sepsis care; provides clinical and professional expertise for patients, families and staff members; collects, analyzes, makes recommendations and reports sepsis related data; and, provides sepsis education at the bedside and in the classroom. Assesses barriers and implements Bundle Compliance Strategies.

  • Facilitates the development, implementation and ongoing adherence of evidence-based sepsis recognition and treatment protocols; works with the medical staff for adoption and use of sepsis PowerPlans.
  • Co-chairs the Sepsis Multidisciplinary Team meetings monthly; includes agenda preparation, review of minutes and status of past actions, forwarding of sepsis team reports to the Quality Committee and applicable medical staff department meetings (ED, Medicine, Hospitalist, Critical Care, MEC, etc.)
  • Collects and analyzes sepsis program patient data to assist with documentation, coding, clinical care, and improved outcomes; includes providing education, coaching and clinical expertise to the care team, patients and families.
  • Participates in rounding ensuring timely sepsis recognition and compliance with the sepsis bundle; pro-actively responds to patient needs to improve patient outcomes overall patient experience.

Qualifications

 

  • Minimum of three (3) years healthcare experience in an acute care, critical care and/or ED experience with a solid grasp of hemodynamics
  • Bachelor's degree in nursing with a Master's degree preferred
  • Competency in Outlook, Word, PowerPoint and Excel
  • Familiar with MIDAS
  • ANCC, CNRN, SCRN, or AACN certification in specialty area preferred
  • Must have analytical skills for collecting and analyzing data
  • Must thoroughly understand the CMS Sepsis Bundle (SEP-1) core measure requirements and remain current on all changes and updates
  • Must be comfortable communicating, coaching and educating staff and providers.
  • Ability to be an active, effective team member.
  • Excellent interpersonal and telephone skills.
  • Ability to handle care issues that are confidential, sensitive and/or controversial. Must be skilled in the management of multiple tasks and project management.
  • Current RN License, Current BLS and ACLS certifications
