Responsibilities

Job Summary / Purpose

This role leads the development of national Population Health care model innovation with supporting data, workflows and implementation advisory services. Reporting to the System VP for Value Based Care Clinical Strategy, the System Director will lead a small team in developing, improving, communicating and replicating care models to support population health throughout the enterprise. The System Director leverages expertise as a physician, knowledge of value based care and strong analytical and financial acumen to assess current state and partner with stakeholders in adopting care models that will move them forward to thrive in a value based environment.

The System Director works with enterprise executives as well as front line clinical and operations staff, designs workflow and performance management, evaluates care model effectiveness and looks ahead to mitigate risks. Ensures clear definition of success measures, actively collaborates to align work with clinical, provider and patient experience and financial targets. Is able to communicate and promote care models effectively to a variety of audiences and stakeholders. Will play a key role in identifying and managing partnerships to accelerate innovation.

Essential Key Job Responsibilities

  • Leads efforts to position CommonSpirit, and Enterprise Population Health specifically, as a “learning organization,” working across IT, analytics, clinical strategy, and external partners to build a robust implementation science and evaluation function, with the goal of driving more precision in program efficacy to support both operational performance and publications in care delivery and innovation literature
  • Possesses keen understanding of care models that address Social Determinants of Health; aligns work with the CommonSpirit Health Transformative Strategy: Care for Vulnerable Populations.
  • Takes high level goals and provides definition, structure and phasing to for broad deployment.
  • Collaborates with cross functional resources to develop dashboard for KPIs related to key initiatives.
  • Engages and collaborates with internal and external subject matter experts to identify best practices and understand local dynamics for the design and deployment of care model playbooks.
  • Supports enterprise initiatives with data collection/analysis, research, and stakeholder engagement.
  • Collaborates on publications and press releases.

Qualifications

Minimum Qualifications: 

  • Advanced Degree
  • MD or DO Licensure/Certification
  • Knowledge of value based care and population health. Experience in care delivery transformation.
