This role leads the development of national Population Health care model innovation with supporting data, workflows and implementation advisory services. Reporting to the System VP for Value Based Care Clinical Strategy, the System Director will lead a small team in developing, improving, communicating and replicating care models to support population health throughout the enterprise. The System Director leverages expertise as a physician, knowledge of value based care and strong analytical and financial acumen to assess current state and partner with stakeholders in adopting care models that will move them forward to thrive in a value based environment.
The System Director works with enterprise executives as well as front line clinical and operations staff, designs workflow and performance management, evaluates care model effectiveness and looks ahead to mitigate risks. Ensures clear definition of success measures, actively collaborates to align work with clinical, provider and patient experience and financial targets. Is able to communicate and promote care models effectively to a variety of audiences and stakeholders. Will play a key role in identifying and managing partnerships to accelerate innovation.
